01 / Who We Are

The name and
what it means.

Pimâtisiwin Transitions Incorporated is a Manitoba-based health consulting firm founded in May 2026 to address a gap that quietly drives poor outcomes across the province: what happens when a patient leaves the hospital.

The name begins with the Cree word Pimâtisiwin — pronounced pee-MAH-tih-see-win — which means life in its fullest sense: living well, in balance, in relationship to land, family, and community. We named the practice for it because every transition in care is, at its heart, a transition in someone's life.

Whether the question is a hospital in Thompson working to reduce avoidable readmissions, a regional health authority designing its first dedicated discharge lounge, or a community organization building environmental health literacy in a remote First Nations community — the underlying work is the same: making the moment of transition safer, more humane, and better supported.

We are small by design. This is a focused practice, not a large firm with generalist consultants. Every engagement is led by the founder and shaped by direct, expert involvement. That is not a constraint — it is the point.

Evidence first

Recommendations grounded in the discharge and patient-flow literature, in measurable outcomes, and in the operational data of the organization we're working with — not in management trends or received wisdom.

The patient is a person

A discharge is not a bed-management transaction. It is a person leaving care — often frightened, often in pain, often without adequate support. We design processes and spaces that hold that truth at every step.

Northern & rural understanding

Distance, transport, weather, language, kinship networks, and cultural trust. We work with the conditions of care across Manitoba — not only those of urban tertiary centres — because that is where many of the hardest problems live.

Practical delivery

Plans your team can actually execute on Monday morning, written in clear language, sized to the resources you have. The best plan that never gets implemented is not a good plan.

Humility in practice

We are guests in every organization we work with. Staff knowledge, patient experience, and institutional history are resources to draw on — not obstacles to be overcome. We listen before we advise.

Dr. Shoyemi founded Pimâtisiwin Transitions Incorporated in May 2026, bringing focused, evidence-based consulting to the part of the health system that too often goes under-attended: the discharge — the threshold between hospital and home, where outcomes are quietly won or lost.

His practice draws on work in health promotion, environmental health education, and care-transitions advisory, with a particular interest in how the design of physical environments — discharge lounges among them — shapes patient experience, hospital flow, and clinical safety.

Based in The Pas, he brings both professional expertise and lived understanding of the Northern Manitoba health context: the transport barriers, the extended distances from tertiary care, the strength of community and kinship networks, and the particular trust that must be earned before effective health promotion is possible.

Every engagement with Pimâtisiwin Transitions is led directly by Dr. Shoyemi. There are no junior consultants standing in for principals. Clients receive the full weight of his expertise and attention, from first conversation to final deliverable.

We work on
Treaty 5 territory.

Our office is in The Pas, Manitoba, situated on Treaty 5 territory — the traditional lands and homeland of the Cree, Anishinaabe, Dakota, Dene, and Métis peoples. We acknowledge this with respect and with recognition that the work of caring for people in transition is older than this corporation, and far older than the health system we advise within. We are guests in it, and we hold that responsibility seriously.

This acknowledgment is not a performance or a formality. It shapes our approach to community health engagement, informs our understanding of Northern health inequities, and reminds us that the communities we serve have their own knowledge, their own ways of caring for people, and their own definitions of what a good life looks like.
